Requirements

• Basketball Experience: Please describe your experience
• Photos: Minimum of 2 recent color photos (1 close-up and 1 full body standing photo). Photos must be current, unfiltered, and feature only you. No sunglasses, hats, or heavy editing. Include a photo in sports attire suitable for basketball.
